Defect Report and 5 Why Analysis Template
An Excel template for a defect report and root cause analysis, built on facts first, analysis second (事実 → 分析 → 対策). The first sheet follows seven parts: summary; the five facts to report as soon as a defect (不具合) occurs (what, when and where, how many, what temporary action is in place, cause or 'under investigation'); a problem statement narrow enough to investigate (what, where, when, extent, impact, expected, known boundary); a 5 Why table with evidence and a hypothesis-or-proven status; an action table split into containment, correction, corrective action and preventive action, each mapped to a proven cause; verification and effectiveness review; attached evidence. The sample 5 Why chain comes from the source article and stays marked as a hypothesis, because 5 Why is a question, not proof. The guide sheet separates symptom, failure mode and cause, explains the four kinds of action, lists questions to ask instead of stopping at 'operator error', gives a 15-point root cause checklist for automated machines, report-writing principles and reporting mistakes.
